-          Camp Green Dog Foundation reserves the right to request a home visit before and after an adoption is approved.

 

-          I understand that Camp Green Dog Foundation has the right to refuse or rescind an adoption.

 

-          I understand that Camp Green Dog Foundation provides age appropriate vaccinations but that subsequent booster and rabies shots are my responsibility.

 

-          I understand that all animals will be spayed or neutered prior to adoption.  Exceptions can only be made with the approval of the CGD Foundation Board.

 

-          I will provide regular veterinary care for this animal including: yearly annual exams, heartworm prevention, flea and tick control, booster vaccinations, and emergency care if needed.

 

-          If the adopted animal is outside for any extended period of time, protection from the elements and fresh water will be provided.

 

-          I understand that this is an adoption agreement, not a sale.

 

-          I understand that I cannot sell, transfer, trade, or give away the adopted animal without the approval of Camp Green Dog Foundation.

 

-          I understand that I am legally obligated to return the adopted animal to Camp Green Dog Foundation if I am no longer able to provide care.

 

-          I understand that the adoption fee is non-refundable.

 

-          I certify that the information I have provided on this adoption application is correct, and that false information may result in the cancellation of this adoption.
